CROSS(Chemoradiotherapy for Oesophageal cancer followed by surgery study) published in 2012 proved to be a practice changing study treatment of locally advanced esophageal located at the middle and lower third esophagus. When chemoradiotherapy became standard care cancer, we, tertiary centers India struggled bring this concept home because various obstacles low income countries like ours. Limited infrastructure, overpopulation, financial issues, ignorance lack strong multidisciplinary team are stumbling blocks. Most ardous task is convince patients go so called food pipe, specially after symptomatic relief neoadjuvant chemoradiation treatment. Retrospective done Department Radiotherapy, centre Northern India. All histopathology proven cases carcinoma esophagus registered our department from january 2019 December 2021, eligible CROSS protocol were taken study. Total 246 January 2021 which 85 non metastatic, disease involving or protocol. 47(55.3%) denied any form surgical intervention begining given definitive chemoradiation. 29(34%) refused treatment, only 9(10%) completed whole We found that out all 10% could complete including surgery. trial established benchmark but real-world data shows providing optimal still tough row hoe reasons extending unwillingness undergo major economic crisis MDT several centres. Better acceptable protocols with organ preservation needs studied adopted have high burden infrastructure.
